Why do we auditors find so many assessment forms that that are repetitive and frankly, wasting your time while not even collecting the required information? I think it is because people tend to you use any old intake or assessment form. This isn't serving you well.
Wouldn't you prefer an intake form that is designed for your service as opposed to someone else's service? This session will be a hands on experience where we will workshop a 'Participant Intake' form for you to take away with you and use in day to day service.
