Addressing Chronic Disease Chronic disease covers a wide range of chronic and complex conditions. Chronic diseases contribute heavily to poor health and well-being outcomes for participants and clients. The Session Chronic disease can go un-noticed unless people are educated about: - How to Identify the person’s chronic disease conditions and how these will impact everyday situations. - How to determine the best support plan and pathway to care for, and support a person with a chronic disease, who to involve and how to make a referral. - How to determine the level and type of support and assistance required by the person within the context of disability or home care services. - How to take steps to adjust the services and support to meet the specific need. - Actively involving the person in the development of strategies to self-manage their chronic disease. - Discussing with the person, their family and other carers, the full range of issues that could have an impact on their well-being. - Responding to the range of issues that present, in an integrated way. - Supporting the person to understand their chronic disease condition. - Facilitate the person to self-manage (where possible) through the provision of advice and resources. - Identify and discuss any unmet needs and requirements and make an appropriate referral.
